  1. Turn on a ruler from View tab.
  2. Set a right tab stop (marker) at 1 1/2" location.
  3. Set a left tab stop (marker) at 2 1/2" location.
  4. Type information shown below.
  5. Fill in dots between two tab stops.
  6. Apply a border with background color.
